Simple Choice / Bundles

Simple Choice / Bundles Promotion - Offer and stated rates are available to residential customers only. Subscription to the Triple Savings bundle includes Unlimited Calling, 256Kbps High Speed Internet and DISH Family package from CenturyTel | DISH Network. Stated rate is valid for first twelve (12) months of service, after which standard rates will apply. Stated rates exclude applicable taxes, surcharges and fees. Triple Savings bundle does not apply to metered/measured line service, PBX, Key, or Centrex accounts. By accepting a bundled service offering, you agree to purchase such bundled services from CenturyTel for a minimum period of twelve (12) months. Early termination will result in you being charged and responsible for payment of the applicable monthly recurring service fee multiplied by the number of months remaining in the minimum service period. Your discontinuation of any portion of the Triple Savings bundle will result in you being charged standard rates for remaining services. Your Social Security number may be used to obtain credit scores and will not be released to third parties except for verification and collection purposes or if required by applicable law, regulation, and/or legal or regulatory process. In some cases, an advance deposit may be required.

Unlimited Calling - Depending on your service area, toll and EAS may or may not be included in the stated rates, and may or may not be mandatory charges. Eligible features in the Triple Savings bundle may vary by market. You may dial *67 (touchtone) or 1167 (rotary) prior to placing a call to block your calling information. To subscribe to the Unlimited Long Distance plan, you must choose CenturyTel Long Distance, LLC as your IntraLATA and InterLATA toll carrier. You must purchase CenturyTel Long Distance service per the terms of applicable tariffs. The Unlimited Long Distance plan is for typical domestic residential voice usage only, and cannot be used for any purpose inconsistent with typical domestic residential voice usage. If usage under this plan is not consistent with typical residential customer usage, as determined in CenturyTel's sole discretion, CenturyTel reserves the right to move you to an alternative plan. Further, calls that do not fall within the Unlimited Long Distance plan include but are not limited to fax and data calls (billed at 10 cents per minute; includes calls to long distance dial-up Internet providers), multi-party conference calls, calls to 900 numbers, directory assistance, calling card, operator services, international calling, toll free calling services, calls used for general business purposes, commercial facsimile, auto-dialing, resale, chat rooms, transcription services, call centers and direct telemarketing centers. Long distance plan rates apply to direct dialed U.S. calls including Alaska, U.S. Puerto Rico, Guam and U.S.V.I. CenturyTel Long Distance, LLC reserves the right to cancel, modify or discontinue the Unlimited Long Distance plan at any time without notice. OTHER RESTRICTIONS APPLY. High-Speed Internet - Phone line is required for CenturyTel High-Speed Internet (HSI) service. HSI service has minimum computer system requirements. Because downloading speeds, reliability, and level of security are at times dependent upon the Internet, downloading speeds, reliability, and level of security may vary. Speed and service may vary by area. CenturyTel is not responsible for any long distance charges incurred; refer to your calling plan to see if charges will apply. PC Security Suite is free for first three (3) months of service, after which PC Security Suite will result in a charge of $3.95 per month. If you do not wish to keep PC Security Suite and pay $3.95 per month, you must call CenturyTel's Customer Service Center to cancel the service. CenturyTel | DISH Network - Offer is available in the continental United States for new, first-time CenturyTel | DISH Network residential customers. All prices, packages and programming are subject to change without notice. Local and state sales taxes may apply. Where applicable, equipment rental fees and programming are taxed separately. All DISH Network programming, and any other services that are provided, are subject to the terms and conditions of the promotional agreement and Residential Customer Agreement, available at www.dishnetwork.com or upon request. Long Distance

EasyTalk Long Distance Promotion - Plans using a bucket of minutes, per minute rates apply to minutes over the bucket, unused minutes cannot be carried over to subsequent month(s), and the full dollar amount for each bucket is not prorated in the first month of the billing. Long distance services are provided by CenturyTel Long Distance, LLC. To receive long distance plan rates, you must choose CenturyTel Long Distance, LLC as your IntraLATA and InterLATA toll carrier. You must purchase CenturyTel Long Distance service per the terms of applicable tariffs. Additional costs may apply for operator services, international calls, directory assistance, calling card rates and payphone surcharges. Long distance plan rates apply to direct dialed U.S. calls including Alaska, U.S. Puerto Rico, Guam and U.S.V.I. CenturyTel Long Distance, LLC reserves the right to cancel or discontinue the EasyTalk long distance plan at any time without notice. Stated rates exclude applicable taxes, surcharges and fees.

Plan Information - You are eligible for the rates available under a Residential Long Distance Plan only if you are a residential customer. Your plan also may require that you agree to the terms and conditions of a service agreement for a minimum term. Refer to the plan description to find out if your plan requires a service agreement. If you terminate your plan prior to the expiration of the minimum service term, you may incur early termination charges.

Savings - The plan selected by you may or may not result in savings to you. Your savings will depend on your market, usage, prior carrier(s), and the plan selected.

Prices and Rates - The long distance plans and rates are subject to change, may vary by service area, and do not apply to metered/measured line service, PBX accounts, or Centrex accounts. The rates under each plan apply only to direct dialed U.S. calls. You may incur additional charges when you dial the operator, an international number, or directory assistance or when you dial with a calling card or from a payphone. Plans are subject to change. Plan rates exclude applicable taxes and fees. Some restrictions apply.

Long Distance Service - To receive long distance plan rates, you must choose CenturyTel Long Distance, LLC as your IntraLATA and InterLATA toll carrier. You will be selecting CenturyTel Long Distance, LLC as your long distance carrier by selecting one of these long distance plans. A National Access Fee of $1.99 applies. Long Distance services are provided by CenturyTel Long Distance, LLC.

Blocking Your Telephone Number - You may dial *67 (touchtone) of 1167 (rotary) prior to placing a call to block your information from being passed on to the person you are calling. Applicable taxes, surcharges and fees will apply.

Additional Line

Additional Line - Each additional line has a monthly service charge associated with it and there may be a connection charge. Special offers related to additional lines do not include installation charges associated with the installation of new jacks. Local, state, and federal taxes applicable. Rates for additional lines will vary by service area and are subject to change. Taxes and activation fee may apply.

Additional Line Promotion - Free Additional Line Installation connection provides free service order, line connection and premise visit work to make the line operational to existing jacks. Monthly charge applies to the Additional Line. Long Distance, taxes, fees and other charges not included in free first month.

CenturyTel's provision of, and your use of, the Telephone Protection Plan is subject to the terms and conditions of the promotional agreement and CenturyTel Telephone Protection Plan Terms & Conditions, available upon request. Certain equipment may not be eligible for Inside Wire Maintenance with Telephone Protection. Inside Wire Maintenance is an optional service, not required for basic telephone service, and may be canceled at any time without penalty. You may select Inside Wire Maintenance as a service without subscribing to the Telephone Protection Plan, but must subscribe to Inside Wire Maintenance in order to be able to subscribe to the Telephone Protection Plan.
